Community Health Improvement Plan
Every five years, local public health departments do a Community Health Assessment (CHA) to see what areas are going well for community health and what could be improved. Information is gathered from population data, community organization surveys and focus groups. This information guides the next five years of health programming and efforts in the form of a Community Health Improvement Plan (CHIP).
2025-2029 Community Health Improvement Plan
The 2025-2029 Community Health Improvement Plan was developed in partnership with stakeholders in the Broomfield community and approved by Broomfield's Board of Health. Priorities
The Broomfield community identified Mental Health and Substance Use, Housing Quality, and Healthcare Affordability and Preventative Care as the priorities for the 2025-2029 Community Health Improvement Plan.
Overview
The goal of this priority is for all Broomfielders to have the opportunity to attain and maintain a state of mental well-being allowing them to realize their potential, cope with life’s stresses, and thrive in our community. This priority was the focus of the 2020-2024 CHIP, where significant progress has been made and momentum continues.
Objectives
- By 2029, increase awareness and understanding of mental health and substance use among Broomfield residents in order to help reduce the stigma and discrimination associated with behavioral health diagnosis and seeking treatment.
- By 2029, increase access to behavioral health care services among Broomfield residents to help people manage conditions that would otherwise limit their social and economic participation.
- By 2029, increase awareness of the importance of social connection within Broomfield to foster a healthier, safer community, ultimately leading to improvements in positive mental well-being.

Get Involved
BPHE's aim is to continue to reduce stigma, increase access to care, and increase connectedness in the Broomfield Community. The settings are in Community-Based Prevention, School-Based Prevention, Health Communications and Behavioral Health Systems.
The Broomfield Behavioral Health Coalition is comprised of over 50+ stakeholders in Broomfield who represent a wide range of partners, including behavioral health providers, health systems, schools, nonprofits and community representatives. The Coalition was formed to provide guidance on what strategies are most effective in addressing the goals of the Behavioral Health Improvement Plan, as well as oversight of Broomfield's opioid settlement funds. Regional Opioid Council and Broomfield Behavioral Health Coalition
The Regional Opioid Council and Broomfield Behavioral Health Coalition brings together community organizations and community members to inform the work of the public health department in addressing behavioral health in Broomfield. The Coalition exists as a group that can learn and benefit from one another to collectively improve the behavioral health of Broomfield.

Overview
The goal of this priority is to promote safe and attainable housing for all Broomfield Residents.
Objectives
- By 2029, BPHE will increase knowledge and understanding in the community that stable, attainable housing is central to the health of Broomfield residents and families.
- By 2029, improve housing quality by providing resources to mitigate health impacts.
- By 2029, improve access to housing assistance programs and improve provider efficiency by enhancing and integrating referral systems

Overview
The goal of this priority is that all Broomfielders have awareness of and the opportunity to access preventative health care services.
Objectives
- By 2029, increase knowledge and connections in the community related to the importance of preventative health care and how to access it.
- By 2029, reduce barriers to access by strengthening the local provider network’s capacity to provide accessible and culturally responsive care.
- By 2029, increase the number of Broomfield residents accessing recommended preventative health care (pre/post natal care, well visits, screening for mental health needs, etc).
