Family Connects Nurse Visitation Program for Families with Newborns

Family Connects offers a nurse visit to all Broomfield families with a newborn who has been delivered at a Boulder County hospital including foster parents, adoptive parents, refugee families, families who already have other children, and parents experiencing a miscarriage or loss of a newborn. A nurse visits your home from three to twelve weeks after birth to check on the parent, baby and the whole family. There is no additional cost to you. 

Eligibility

Broomfield families who have a baby born at a Boulder County hospital are eligible:

  • Boulder Community Health
  • Avista
  • Good Samaritan
  • UCHealth Longs Peak
  • Longmont United

Family Connects is also available in Arapahoe, Boulder, Denver, Eagle and Jefferson Counties. To review eligibility criteria for these counties, go to https://familyconnectscolorado.org/.

Services

Every family needs support after bringing home a baby. The needs are different in each family – from help with feeding and safe sleep to getting information about child care and parenting groups. Family Connects links parents to the individual community resources they may need need, like:

  • Baby weight check
  • Safe sleep information
  • Infant feeding and fussiness
  • Parent’s health check
  • Breastfeeding support
  • Postpartum depression screening
  • Scheduling doctors appointments
  • Understanding child care options
  • Community connections
