Applications

Requests for certified copies of birth and death certificates may be made by mail, online, or in person.

This is a walk-in location and clients are served first come, first served. Wait times may vary based on current client volume. 

Death Certificate

  • Available for individuals who died in Colorado between 1975 to present, regardless of the county in which they died
  • Cost: $25.00; each additional copy $20 (on the same order)
  • Certificate Corrections: $25.00; Certificate Exchanges: $20.00

• Note: newborn birth certificates take 10-14 days to process. Please call to check if your newborn's birth certificate is available before coming to the office.

Ordering

Order by Mail 

Order through the mail by including a legible copy of the applicant’s state identification or driver’s license with a completed and signed application and payment. Applications can be downloaded and submitted by mail to Health and Human Services, 100 Spader Way, Broomfield, CO 80020.

Order Online

For your convenience, you can process requests for birth certificates through CDPHE or by phone at 866.687.1463. An additional fee is charged by VitalChek for using this service. Proof of relationship and a valid photo identification are required to process all requests. 

In 2026, Colorado will observe two significant anniversaries: America 250 and Colorado 150. Upon request for all birth certificates issued in 2026, you can have the Colorado 150 logo added to the birth certificate at no charge.
