Getting a Passport

The City and County of Broomfield is a U.S. Department of State designated Passport Acceptance Facility. The Community Assistance Center at the George DiCiero City and County Building, One DesCombes Drive, will assist with passport applications. Appointments are REQUIRED for processing passport applications.  A maximum of 4 applications can be processed at each appointment.  Passport appointments open up two weeks at a time.  For reference, passport appointments will be opened after 12 noon according to the following calendar:

How to Apply

To apply for a passport, please bring the following:

  • Completed DS-11 form, filled out in black ink, but not signed.
    • Obtain a DS-11 Form online at travel.state.gov
  • Personal identification - one of the following:
    • driver’s license, military ID, student ID, work ID, foreign passport, etc. 
    • Additionally, please bring a photocopy of your ID on plain 8 1/2 X 11" white paper, showing the front and back of ID on same side of one page.
  • Citizenship evidence- one of the following: a state-issued birth certificate*, naturalization papers, an adoption certificate, previous U.S. passport, etc.
  • One passport photo- (available, for example, at Walgreens or Walmart) passport photo requirements are 2 by 2 inches in size, front view-full faced, white or off-white background preferred, recently taken within the last 6 months, printed on photographic paper. No photo will be accepted with uniforms or items resembling uniforms of any kind (no camouflage, epaulets, etc.), no glasses, no hats or other headgear that obscures the hair or hairline. Headgear worn daily for religious purposes is acceptable as long as the applicant provides a signed statement to that effect. The photo must still show the full face of the applicant.
  • All applicants must provide their social security number on their passport applications.

*California Birth Abstracts or Texas Short Form Certificates are no longer acceptable. Please call for information regarding this issue.

Renewals

If you have a previous passport, you may not need to apply in person. You MUST renew by mail if all of the following are true:

Your most recent U.S. Passport:

  • Is undamaged and can be submitted with your application;
  • Was issued when you were age 16 or older;
  • Was issued within the last 15 years; and
  • Was issued in your current name or you can legally document your name change with an original or certified copy of your marriage certificate or the government-issued document evidencing your legal name change.

If any of the above statements do not apply to you, you must apply in person.

Fees

Passport Book

The U.S. Passport Book may be used for any type of international travel.

Passport Card

The U.S. Passport Card may only be used for re-entry into the United States at land borders and sea ports-of-entry when traveling from Canada, Mexico, the Caribbean and Bermuda. Passport Cards may not be used for air travel.

Only one execution fee is charged per application when applying for both the Book and the Card at the same time.

Passport fees are payable only by check or money order to U.S. Department of State. Execution fees are payable by check, money order, cash or credit/debit card to City and County of Broomfield.

Receiving Your Passport

Please check travel.state.gov for current information about passport processing times.  Adult passports are valid for ten years. Minor passports are valid for five years.

To expedite an application, an additional fee of $60 will be required.  For current information on expedited passport processing times, please check travel.state.gov.

Children Under 16

To apply for a passport of a child under the age of 16, both parents or legal guardians must appear with their child. If the second parent is not available to sign, the appearing parent must present evidence of sole legal custody of the child or a notarized Statement of Consent showing written consent from the non-appearing parent and a copy of the front and back of the non-appearing parent's photo I.D. Proof of U.S. Citizenship (i.e. birth certificate) must also be submitted even if the child has been issued a previous passport.
