1095 Forms for Tax Year 2025

In its Instructions for Forms 1094-C and 1095-C for 2025, the IRS has indicated that employers no longer have to automatically send Form 1095-C to individuals.

If you are a current employee, you received email instruction on how to request your 1095-C for 2025.  If you are a former employee, please email hr@broomfield.org to request your 1095-C form.  If you are a member of Kaiser Permanente, you will receive your form 1095-B directly from them.

We encourage current and former employees to review the IRS’s FAQ on health care information forms for individuals to learn more about the purpose of the Forms 1095-A, 1095-B and 1095-C.

If you have any questions, please contact benefits@broomfield.org.
