Collaborative Management Program Transitioning to Broomfield FISH

The Collaborative Management Program (CMP) brings together agencies and services designed to improve outcomes for at-risk, high systems-use children, youth and families in Broomfield County. Broomfield’s CMP values family voice and adheres to best practices that promote family engagement and data-informed processes.

In Broomfield, the CMP has historically been located within Broomfield Human Services and, through collaboration and assessment of the program, is now transitioning to Broomfield FISH with a Memorandum of Understanding being issued in June. The program has been revamped to be more community facing and to better support the intention and effort for the program to be accessible by everyone.

The CMP Coordinator, which has recently become a full-time position, will now be located at Broomfield FISH in close partnership with the CMP Executive Team, which includes representatives from Broomfield Human Services, Safehouse Progressive Alliance for Nonviolence, Mental Health Partners and FISH. 

The two primary goals of the Collaborative Management Program are:

  • to support youth and their families who are experiencing adverse life events through facilitated  problem-solving and community-based, wrap-around care and programming
  • to identify gaps in services within the community and work with stakeholders to build solutions and fill unmet needs
