Filing a Claim/Incident Report

Instructions for City and County Employees

Please note: this does not include workers compensation claims. Workers Compensation is handled by HR and that information can be found at Safety and Workers Compensation. Employees should still report all accidents to their supervisors and if the accident involved a City and County vehicle, please call Fleet Services at 303.438.6336 or email Public Works at publicworks@broomfield.org.

Instructions for the public: Claims/Incident Reports should be filed as soon as possible, please fill out the Claim Incident Report to submit your report (you may attach any documentation that you have available). Please note that note that the City and County of Broomfield is not promising payment and that you should also contact your insurance company for consideration.
